1. By completing the form and entering the prize draw, you are agreeing to these terms and conditions.
2. The prize draw is being run by New Leaf Health Ltd (NLH).

Eligibility to Enter

3. This draw is open to entrants over the age of 18 who have engaged with a health check by New Leaf Health.
4. Employees and subcontractors of New Leaf Health are excluded from the draw.
5. In entering the draw, you are confirming that you are eligible to do so.
6. Maximum of one entry per individual per 3 month period (beginning at the first entry date).
7. The draw is free to enter, but entrants must have completed a recent health check with New Leaf Health and be completing the feedback form in good faith.

How to Enter

8. To enter, entrants must complete the health check feedback form hosted on this page following a recent workplace health check with New Leaf Health.
9. NLH will not accept any responsibility for incorrectly entered or incomplete contact details.

The Draw

10. The draw will run for each calendar month, with one winner selected per month.
11. Duplicate entries, or entries otherwise considered to not be genuine or in bad faith will be removed from the draw.
12. A draw will only take place if a minimum of 20 genuine entries have been accumulated. Entries below this threshold will be rolled over into the following month. If necessary, this process will continue until the minimum threshold has been reached.
13. The draw will be made by random selection and not based on any answers given in the questionnaire.
14. NLH reserves the right to disqualify any entrant who is believed to be entering in bad faith or dishonestly.

The Prize

14. The prize will be one £20.00 Amazon.co.uk voucher per month.
15. No alternative or cash prizes are available.
16. The voucher is only available in GBP.
17. We reserve the right to substitute the prize for an alternative of equal or higher value if circumstances beyond our control make it necessary to do so.
